Background : Undifferentiated embryonal sarcoma of the liver (USEL) in adults in an exceedingly rare but aggressive mesenchymal tumour of the liver, classically associated with an abysmal prognosis of less than a year following diagnosis. Due to its diagnostic challenge and rarity, management for UESL in adults is unfortunately usually extrapolated from the pediatric population.

Methods : We performed a retrospective case series analysis of three cases of USEL in adults in managed in a tertiary institution with expertise input from a dedicated cancer centre. Patient demographics, clinical presentations, treatments and survival outcomes were analysed. The study was performed in a tertiary specialised institution in Singapore. The results and analysis are presented as a descriptive analysis of a case series.

Results : 3 adult patients with histology proven undifferentiated embryonal sarcomas of the liver were included. The ages ranged from 20 to 32 years. Two patients underwent definitive surgical resection and adjuvant chemotherapy and remained disease free thus far. One patient presented with metastatic disease on diagnosis and died 5 months after receiving a trial of palliative chemotherapy.

Conclusions : The results of the present study suggests that margin-free surgical resection with adjuvant chemotherapy as the treatment regime may offer optimistic survival outcomes in adults.
