Background : Teratomas are germ cell tumors occurring usually in gonadal organs. Teratomas are neoplasms usually found in the gonadal organs: ovaries and testis. Extragonadal teratomas locations are uncommonly reported. The majority of teratomas are found in the pediatric age group. Furthermore, teratomas originating in the liver are very exceedingly rare. Specific signs of teratoma on ultrasound, (CT) or (MRI) are lacking, which makes hitopathologiacl study the only diagnostic tool.

Methods : We report a 59 years sold female with presented of a solid mass on left liver lobe. Contrast-enhanced CT scan revealed a solid round heterogenous lesion suspicion as liver metastase tumor. The patient planed for hepatectomy and radical hysterectomy. The patient have a normal liver function tests. The diagnosis of primary mature teratomas was confirmed on histopathology with the tumor of uterus a leiomyoma uteri.

Results : Patient underwent an elective liver segmentectomy 2,3 and radical hysterectomy. Intraoperative sonography study revealed a hypoechoic mass suspected as hepatolithiasis. Sample of left liver segment showed a calcification on canaliculi. A definitive diagnosis of mature teratoma is confirmed by histopathology study, while tumor on uteri revealed as leiomyoma. Patient was on intensive care unit post operative and treated on the ward on post operative days 5th.

Conclusions : Primary mature teratomas originating of liver are very rare case with a few study literature reported. Imaging diagnostic study have no specific for diagnosis teratoma of liver. Histopathological study is the only diagnostic tool for mature teratomas of liver with segmented or total hepatectomy still remain the definitive surgery.
