Background : After two years of pandemics, our team was relocated to a new hospital – The Bucharest Institute of Oncology and started an hepato-biliary-pancreatic surgery department, in a place with no history in advanced HBP surgery. After nine month of woking in the new hospital, we are still trying to create a routine for complex HBP cases.

Methods : We performed a case match study of 5 cases of pancreatic head adenocarcinoma and 5 cases of extrahepatic cholangiocarcinoma performed in 2019 in the experienced center where we worked at the time, with 10 cases of similar pathology performed in the new department. Prior starting more complex surgery than they were used to, a mindset training workshop for the department nurses was performed.

Results : The results showed were no relevant differences regarding postoperative course as well as pathology reports. However, a much longer hospital and ICU stay were observed in all the patients treated in the new department. The longer ICU stay was caused by precautions rather than objective reasons

Conclusions : As proved before in studies, an experienced HBP team can replicate it’s results in a low volume center. Training the mindset of the stuff is an important tool for mantaining a good postoperative course, as well as a better patient surveillance, especially for difficult cases.
